Using your insurance plan

If you want to use your insurance plan, you pay the fee directly to me and I will provide a receipt and any other documentation needed for you to submit to your insurance company for fee reimbursement directly to you. In order to find out how much reimbursement you would receive, contact your insurance company and find the answers to the following questions:

  • Do I have insurance coverage for outpatient mental health?
  • Is pre-certification required by my primary care physician before services will be covered?
  • Do I have an out of network deductible that must be met
    before reimbursement begins?
  • How much do you pay per psychotherapy session (cpt code 90834) for an out of network psychologist?
  • Is there a limitation on how much you will pay per session?
  • How many therapy sessions per year does my plan allow?
  • Are there requirements for the psychologist to periodically re-certify the need for continued therapy?
  • Are there particular forms that I or my psychologist must fill out to obtain reimbursement? (Please obtain these forms)

Payment & Fees

Cash, check and credit cards (Visa or MasterCard) are accepted for payment. Individual sessions last 45 minutes and cost $175.


Cancellation Policy

Cancellations made after appointments are scheduled will be charged full fee, unless the appointment can be rescheduled within the same work week. Cancellation fees are not reimbursed by insurance companies.
