If you want to use your insurance plan, you pay the fee directly to me at each session and I will provide a receipt and any other documentation needed for you to submit to your insurance company for fee reimbursement directly to you. In order to find out how much reimbursement you would receive, contact your insurance company (behavioral health section of benefits information or via phone) prior to your first visit and find the answers to the following questions:

• Do I have insurance coverage for outpatient mental health?
• Is pre-certification required by my primary care physician before services will be covered?
• Do I have an out of network deductible that must be met 
  before reimbursement begins? How much is the deductible?
• How much do you pay per psychotherapy session (CPT code 90834) for an out of network psychologist?
• For potential AD/HD evaluation, hot much to you pay for an assessment (CPT code 90791)?
• Is there a limitation on how much you will pay per session?
• If the coverage is a percentage of fee charged, is that percentage based on my Doctor's actual fee charged, or on a "usual and customary" amount?  If the latter, what is the standard "usual and customary" amount allowed for outpatient psychotherapy?
• How many therapy sessions per year does my plan allow?
• Are there requirements for the psychologist to periodically re-certify the need for continued therapy?
• Are there particular forms that I or my psychologist must fill out to obtain reimbursement? (Please obtain these forms)
